Serving an overall list size of 80,000 patients, Acton PCN encompasses 14 GP practices today. The team asked Suvera to launch a virtual group consultation service that could support proactive diabetes management, and enable early intervention and prevention across their footprint.
Suvera’s innovative virtual clinic provided diabetes education at scale. Individuals were invited to discuss goals and develop plans in 90-minute sessions. Group clinics also included review of a diabetes results board for recent blood tests, blood pressure, weight/BMI and medication. Clinics were provided in addition to patients’ individual annual diabetes reviews.
