NHS South East London ICS and Suvera establish pharmacist-led virtual care programme to elevate hypertension management

Suvera has partnered with NHS South East London Integrated Care System (ICS) to pioneer a transformative programme to improve hypertension control across South East London borough, Lewisham. The six month program will contribute to addressing health inequalities and enhancing patient outcomes using Suvera’s integrated virtual care solutions.

Suvera has successfully secured a partnership with the NHS South East London ICS, affirming its commitment to deliver robust and innovative healthcare solutions. The forthcoming programme has a primary focus on enhancing hypertension management in Lewisham, an area challenged by low hypertension control rates. Suvera's expert team of clinical pharmacists will implement proactive and interactive care, risk prioritisation, and triaging of remote blood pressure measurements to address these critical health concerns.

The partnership aims to improve patient outcomes by addressing health disparities related to Cardiovascular Disease (CVD), given the significant impact of CVD in England. The initiative specifically targets Lewisham, where some surgeries rank in the bottom 1% for hypertension control and where Black and Minority Ethnic groups, along with people in low income households are disproportionately represented in the uncontrolled hypertension cohort. Together, we're aiming to enhance hypertension control to meet national targets of 77% of patients in control, thereby reducing the risk of heart disease and stroke.

Suvera, who are on the NHS Innovation Accelerator programme, will offer a hybrid virtual service where independent prescribing clinicians carry out virtual reviews for hypertensive patients and offer full end-to-end care management. The service will feature multilingual engagement automations and accessibility features for visual impairment and will concentrate on patients already diagnosed, getting them to target control. Succeeding in these objectives will contribute to considerable prevention of strokes and deaths, reducing the devastating impact this can have on individuals and families and contribute to cost savings across the system.
