Traditional reactive models of care are heavily reliant on in-person consultations, but this approach is becoming increasingly unsustainable. 

Suvera is a CQC registered digital clinic offering a transformative solution to shift the paradigm from reactive to proactive care. Suvera integrates seamlessly to reduce GP workload, and is aligned with the NHS’s long-term goals regarding prevention, digital innovation, and community-based services.

Multiple Channels of Communication

Suvera makes it possible to maintain ongoing contact with patients by offering multiple channels for communication with healthcare professionals. Channels include app-based interfaces, text (SMS), email, and telephone. This ensures accessibility for all demographics including those who are uncomfortable with, or do not have access to, advanced digital tools.

This flexibility reduces barriers to engagement and promotes adherence to care plans.

Suvera’s Digital Clinic Shown to Improve Hypertension Management

As part of our partnership with the South East London Integrated Care Board, we targeted hypertension management across two Primary Care Networks in Lewisham. 

Over a 6 month period in 2024, the programme demonstrated remarkable outcomes with an average reduction in systolic blood pressure of 10.3mmHg per patient. Over 70% of patients achieved control post-intervention, and on average individuals reached a normotensive range within 16 days of their initial invite.

Over 4000 patients engaged with the service. Of these, more than 80% were within the Core20PLUS5 population (socioeconomically deprived or at risk of poor health access and outcomes).

The Lewisham programme illustrates the potential of proactive care to improve health outcomes for all, including those in populations traditionally underserved or at higher risk of poor health outcomes.

Tackling Health Inequalities: Bridging the Gap

Digital health innovations like our virtual clinics hold the promise of addressing health inequalities by making healthcare accessible, affordable, and tailored to individual needs.

Our model actively prioritises local populations where health inequality is more likely, such as Core20PLUS5 populations. This showcases our ability to engage communities that are often marginalised in traditional healthcare settings.

Many healthcare innovations rely solely on apps or online portals, which may widen health inequality. Our clinics reduce digital exclusion by offering phone-based support, so that patients without internet access or digital literacy can still benefit from the service.

This approach not only promotes equity, but also aligns with the NHS’s commitment to reducing health disparities.

Technology plays a central role in our ability to deliver personalised, proactive care. The platform integrates seamlessly with major electronic health record systems such as EMIS and SystmOne, ensuring efficient data sharing and continuity of patient care.

This interoperability reduces administrative burdens for healthcare providers allowing for focused, patient-centred care.

Empowering Patients and Supporting Clinicians

Patients are empowered to take an active role in their health through our remote monitoring tools. They can submit health readings, complete lifestyle questionnaires, and receive tailored feedback – all from the comfort of their homes.

These features enable clinicians to monitor patients’ progress in real-time and make data-driven adjustments to care plans. A patient with hypertension, for example, can regularly upload their blood pressure readings to allow prompt intervention by a healthcare professional if measurements extend beyond a safe threshold.

Patients can choose their preferred mode of interaction or communication. This flexibility not only increases patient satisfaction, but also improves adherence to care plans, leading to better health outcomes.

A study published by BMJ Open highlights that remote patient monitoring has the potential to reduce acute care use if the system is targeted appropriately, designed well, and implemented in a patient and provider-friendly way. Our model exemplifies these findings, combining technology and human-centred design to deliver impactful care.
